[games_access] Survey

Hello, my name is Adam Gourdin, the summer intern at IGDA's office in San 
Francisco.  I am currently attempting to create a broad demographic survey 
for IGDA and would very much appreciate your input.  I would like to know 
(a) what is important to you in terms of diversity issues (b) questions you 
would like to see on the survey and (c) anything else you want to tell me 
that you think is relevant.
