I agree with Mel that the apparent hematologic/immunologic abnormalities 
could be nutritional. Has there been diarrhea in the past or is it 
recent in origin? Is there any previous infection history. Without 
infections it is hard to blame FTT on immunodeficiency. Poor vaccine 
response may be nutritional as well. This child needs Neocate or maybe 
TPN. In a week there should be some response or at least a suggestion of 
a trend. If there is not, then one could pursue a "mega workup".
