I would say that probably no one would be able to get reimbursement to use SYnagis in such a case. The guidelines put out by the RedBook are written in granite. So unless the child has documented lung disease (BPD, interstitial lung disease, not just asthma) and/or cyanotic heart disease it is unlikely to be used.
I have used in for some 22qDS/Digeorge patients with congenital heart disease (even non-cyanotic) who had decreased in T cell # (even with normal Igs) up to age 2 yrs.

This is somewhat related to a question posed a few weeks ago.

Who has used (or routinely uses) Synagis (palivizumab) in children [under
two years of age] who have symptomatic (recurrent upper/lower respiratory
infection) hypogammaglobulinemia [presumbably 'transient']?

Sorry for all the qualifiers - there are so many variables.

Specifically in children who are NOT on immunoglobulin replacement therapy.

If you haven't used it, have you considered it?
