We've been advised red blood cell pits are more reliable than Howell Jolley bodies.

When the patient is old enough I would also recommend assessing response to pneumococcal vaccination as part of the functional evaluation of the spleen. I typically check encapsulated bacteria vaccine titers 2 times a year and re-vaccinate if necessary for my asplenic patients especially if they are unwilling to prophylax.
