Tom, per my response to Joe, actually referring to immune workup.  Sometimes IgG replacement can obscure/"neutralize"(?) pathogenic autoantibodies(?).  I have an OS child who was found to have anti-platelet and ANCA antibodies after developing ITP and renal vasculitis.  Wondering if certain auto-antibodies, if present at diagnosis may predict these complications.  Not seen much in literature.  Not thought to ask in conversations with all of you at CIS, AAAAI, etc.

Depends on the age.  If <12 mos old, serologies are not very useful.  I would just check a urine CMV PCR (more sensitive than blood).  I assume you are talking about inf disease testing, not immune workup.

In an child with suspected combined immunodeficiency (e.g. OS, IPEX, etc), what labs do you find useful to obtain BEFORE initiating IVIG/ScIg as part of pre-BMT work-up/management?
