I have just seen a new patient 68y/o white female with a very long list
of medical conditions that always makes me suspicious as to whether each
one really exists. What caught my attention, however, is a monoclonal
gammopathy (worked up by hematology as indeterminate) with elevated IgG
of ~1,700. Bone marrow has been performed and was normal. I will
obtain kappa/lambda free light chain evaluation, but does any other
pattern of monoclonal gammopathy with significant autoimmunity indicate
more genetic testing? If so, what labs are currently doing these
studies? Thanks to all contributors!
