Thinking of the analogy:  NMDARAb+ encephalitis after HSV1 HSE  is a known entity, but it is clinically rather different from HSE (and HSE recurrences)  and AFAIK never causes HSV-PCR to revert back to positive.

Our lady had had repeated negative PCRs in between episodes, and both PCR (I think 5-6 times if my memory serves me correct) and MRI (twice) have repeatedly become also positive during a recurrence. Thus that it actually is HSV2 that is the culprit seems solid indeed. I would not seek an alternative dg thus. But we did belatedly (after NMDARAbs were discovered) check for these as well, I think at Josip Dalmau's/Philadelphia lab but might have been Stöcker's/Germany instead.

Hi all!

has anyone ever seen a match for my probably ultra-rarest patient:

recurrent HSV2 myelitis (PCR repeatedly +, MRI +)?

Doing well on antiviral long term medication for years now. We have performed WES on her, no low hanging fruits there..
