I have cared for a similar patient with autoimmune polyglandular failure
and autoimmune hepatitis as well as superficial and invasive fungal
infections. She did not have severe lung disease. She received a matched
sibling stem cell transplant about eight years ago. Except for an
episode of bronchiolitis obliterans that responded to glucocorticoids,
CyA and IVIG, she has done well. There have been no new autoimmune
diatheses. Her hepatitis resolved. There has been no endocrine recovery.
Her lung function is normal. If they can be toned up, consider transplant.
