[PAGID] Home IVIG

Chris- According to the 2002 IDF survey (available at www.primaryimmune.org, 2nd patient survey, fig 60, 35% of patients on IGIV received their infusions at home. Note that this was before IGSC became licensed. Lately, with the incresed interest in sub-cu, I think more patients are going to home care by that route. However, because of re-imbursement decisions by CMS (Center for Medicare/Medicaid Services), many Gov't beneficiaries who might have gotten their infusions at home have now been shifted to infusions at hospitals which are classified as having a "Disroportionate share of medicare patients", to whom the supply of IGIV is guaranteed.

If your question is about self/partner/parent IV administration at home, we also were having patients who wre doing this at home soon after IGIV was licensed in this country: see Sorensen, et. al. JACI 80: 810-815, 1987. Interesting coincidence, I just saw one of the patients in that study in clinic last week. He is now about 35 years old and still getting infusions given by his father every three weeks.

I think overall, about half of our IV patients are treated at home, but many patients on Iv in clinic and at home are switching to home sub-cu.

Subject: [PAGID] Home IVIG

Dear Colleagues,

I would like to poll people on home IVIG use. I am typically opposed to it
and have found more recently that insurance companies are requesting it.
During my training, this was never done. Is there are paradigm shift in
facilitating home IVIG that I am not aware of? Thank you for your comments,
Chris
