[CIS PIDD] [cis-pidd] Abatacept usage in LRBA

The abatacept is catabolized similarly to IgG. Because IgG catabolism is
concentration dependent, it would be better to give the abatacept before
the IgG. Probably 1-2 days prior is enough. Alternatively, if you gave the
IgG subcut, there would be no elevated IgG and you could give the abatacept
any time.
