[CIS PIDD] [cis-pidd] Gardasil and Antibody deficiency

Hi Mike!

For the lack of data I doubt if there will be consensus. In vitro tests and their clinical relevance can also be questioned.

For long, I was giving seasonal influenza shots but wondering about their relevance in PADs, CVIDs...

Not overtly scientifically, an anecdotal experience that personally turned me in favor of T cell ag vaccines was that of "swine flu". When H1N1 hit Finland, we got the vaccines only 2 weeks in advance, among the first in Finland. In quick succession we vaccinated all our CVIDs we got hold of.

The first week of H1N1 season was awful, 3 of our "die hard CVIDs" (no pun intended) were hospitalized for pneumonia (usually atypical+lobar superimposed). After that - no hospitalized cases (plenty of prolonged sinusitides though) during the long season...

Anecdotal experience in influenza does not turn into much practical advice for the use of HPV vaccines, but if I had a teenager child with CVID, I personally would vaccinate.

For what it's worth...

Dear colleagues,

The question of whether to give gardasil to our young adolescents with CVID and other primary antibody disorders has come up a few times in our clinic and I'm curious about the group consensus. We have been of the opinion that since it is inactivated, and we have no data regarding whether IVIG provides any protection against HPV, it is unlikely to cause harm, and may potentially induce a cellular response that could be beneficial.

Thanks in advance!
Mike
