[sbe-eas] REQUIRED ETRS FORM 1 by October 4

I was just suggesting some reasons for why the FCC requires a Form 1
without an accompanying national test.  The Form 1 is going to ask for
certain items to be reviewed and attested to, which might otherwise be
overlooked.
