It should be that simple.

OTA broadcasters do not have discreet channels for left/right/SAP, so yes
they provide a different format. And that format is directly compatibile
with the consumer analog receivers (BTSC encoder and SAP generator).

But satellite distributed networks use discreet channels and the cable
headends take those discreet channels as inputs into their modulators.

As for what your obligations may be to the affiliates for them to make their
modifications, you should check your affiliate agreements. That should
spell out what you need to do. It won't spell out the technical
requirement, but it will tell you if you have any obligation beyond
notifying them that the audio is there.

Well, I just can't imagine it's as simple as this:

Secondary Audio Source > ISDN > XLR from ISDN RX to Channel 4 of Satellite
Encoder > Satellite > IRD > XLR from IRD to affiliate provided equipment.

Is it really that simple from our end?

I would assume the affils would have the means to insert the additional
audio channel in our RF channel. Generally speaking, is that something the
broadcaster provides the cable systems or they provide on their own? (or
does it not matter for broadcasters?)

I suppose what I'm asking is, is there anything (hardware), as a content
provider, we would need to provide our affiliates to get them to carry the
SAP?
