I work extensively in the deaf/hard-of-hearing community - having a 45 dB
hearing loss myself! We use actual live persons who can/usually are be at a
remote location - they use a converted stenotype machine to real-time
caption meetings we hold with deaf persons. They're absolutely amazing! We
Skype our conference room table audio to the transcriber - the captioning
comes back in real time via the/an Internet Broadband connection.

Here's a description from one web site:
Realtime Closed Captioning: Realtime closed captions are performed by a
highly skilled stenographer called a realtime captioner. The captioner
utilizes captioning software that formats the realtime text into captions
and sends it via a modem or cable to an encoder.

Any voice recognition software/hardware combo people trust? Most of the
content is live presentations that need to be captioned in real time for HD
closed circuit broadcast.
