[CIS PIDD] [cis-pidd] Osteoporosis treatment in CVID

Colleagues:

I follow a 70+ yo F with CVID, chronic lung disease (40+ pack years of smoking) and severe osteoporosis.
Her endocrinologist wants to put her on Proliatm  (denosumab).  This works by binding RANKL and preventing bone resorption mediated by osteoclasts.

The risk of serious infections in patients treated with Proliatm does not seem to be increased.  However, interrupting the RANK-RANKL axis theoretically may have a negative impact on T-cell and dendritic cell functions particularly.

I would be interested in anyone's experience in treating older CVID patients with this drug.
